Materials needed:

Photo of your model (featuring my crush =P)

A broken glass with black background

Now let’s start with this photo, open it on Photoshop.

Then open the glass one:

Put the glass image above the model layer.

Go to Image>>> Adjustments>>> Color Balance or just press CTRL+B. Color Levels: -100, 0, +100. Midtones. Check Preserve Luminosity.

Then rotate it vertically and resize it to cover the whole space.

Set the glass layer blending mode to Screen.

Create a new layer above the all the layers and fill it with black.

Then go to Filter>>> Render>>> Lens Flare.

Hide this layer first and select on the glass layer. Go to Image>>> Adjustments>>> Curves (CTRL+M). Insert the following values: Output: 0, Input: 96.

Then set the glass layer blending mode to Pin light.

Unhide the lens layer and set the layer blending mode to Screen. Resize and locate it to a new space where you can see it like this one below.

Duplicate the lens layer and set the duplicated lens layer to Linear Dodge (Add), as shown below:

Merge the two lens layer and move it to the center of the chest of the model.

Then go to Image>>> Adjustments>>> Hue and saturation. Put the values: Hue: +180, Saturation: +100, Lightness: 0.

Now go back to the glass layer and duplicate it three times (CTRL+J). Merge the three duplicated glass layers and set the layer blending mode to Screen.

Then go to Filter>>> Blur>>> Motion blur. Put the values: angle:0, distance:45px.

Then go to Filter>>> Sharpen>>> Unsharp mask. Put the levels: angle:13% , radius: 250.0 px, threshold: 250 levels.

Create a new layer above all the layers and fill it with #ff00ea.Then switch the blending mode to Soft Light.

Zoom in the area of the face and use the Eraser  to clean the face.

Zoom out and here is the output:
